This web map was created using ESRI's Flex Viewer download.  It's as easy as Download - Configure - Post to the Web!

 

 

You can start with your own web map by first downloading the source files from ESRI's website here.

 

The next step is to configure the map for your environment.  You'll want to add your logo, point the map at your ArcGIS Server data & probably fiddle with some of the widgets too.  All of this is very easy, all you have to do is edit the config files.

 

The final step is to simply copy the files over to your web server.  Point your browser to <Your web URL>/flexviewer  and you should see your newly configured map!
